FOR DEATH:
- The Insured´s beneficiary or family member, must present himself/herself to the Centro de Negocios de la Mutual, to notify.
- To hand in a letter, asking for the granting of the coverage offered by the policy, related to the credit operation, the death person had with the Mutual.
- Official death certificate issued by the Registro Civil where there is specification of the death´s reason, like the corresponding leaf, sheet number and entry.
- Photocopy by both sides of the death person´s identification card.
- Summary trial issued by the O.I.J. (in case of accidental death), which contains the description of the facts and the forensic laboratory proofs about alcohol (OH) or toxics in blood.
- The beneficiary must sign the authorization form to review the clinical documents where the Insured was attended.
- The beneficiary must indicate in a written way the medical centers where the Insured was attended.
- Photocopy by both sides of the beneficiary(ies) and/or family member who is presenting the claim.
FOR TOTAL AND PERMANENT DISABILITY:
- The Insured, must present himself/herself to the Centro de Negocios de la Mutual, to notify.
- To hand in a letter, asking for the granting of the coverage offered by the policy, in this case by disability.
- Photocopy by both sides of the Insured´s identification card.
- Official declaration of permanent disability issued by an official medical entity well authorized for this effect, which indicates that the disability is given NOT SUBJECT TO REVIEW, the diagnosis and the exact date the disability began.
- Disability declaration from the Magisterio
- Disability declaration from the Poder Judicial (only for the Poder´s employees)
- The Insured must indicate in a written way the medical centers where he was attended.
- The Insured must sign the authorization form to review all the clinical documents of the medical centers where he was attended.
- In case where the CCSS does not grant the pension due to disability, to ask for the document that explains why it does apply. And a “Medical Certification in which is determined the 67% or more of his/her organic or functional ability which does not permit him/her to work.
- Verification of the working status of such injured Insureds at the moment the accident happened: copy of the annual tax declaration. Copy of the services contract or certification issued by the ex-employer.
FOR FUNERAL EXPENSES:
- The Insured´s beneficiary or family member, must present himself/herself at the Centro de Negocios de la Mutual, to notify.
- To hand a letter, asking for the granting of the coverage offered for funeral expenses by the life insurance which the Insured had related to the credit with the Mutual.
- Photocopy by both sides of the Insured´s identification card.
- Death certificate issued by the competent authority.
